Clomid & Enclomiphene for Men | Alternatives to TRT | Everstrong

Men with symptoms of low testosterone often assume testosterone replacement therapy (TRT) is their only treatment option. For some appropriately selected patients, however, medications such as clomiphene citrate (Clomid) or enclomiphene may be considered to support the body's own testosterone production.


Unlike TRT, which supplies testosterone directly, these medications influence the hormonal signaling between the brain, pituitary gland, and testes. That difference may be particularly relevant for men concerned about fertility or maintaining endogenous testosterone production.

Understanding How Your Body Produces Testosterone

Testosterone production begins with communication between the brain and the testes through a system known as the hypothalamic-pituitary-gonadal (HPG) axis.


The pituitary gland releases two important hormones:

Luteinizing hormone (LH) signals the testes to produce testosterone.

Follicle-stimulating hormone (FSH) plays an important role in sperm production and male fertility.


When this signaling pathway is not functioning optimally, some men can have low testosterone despite retaining the ability of the testes to produce testosterone.

This is one reason identifying the potential cause of low testosterone matters. Treatment should be based on symptoms, medical history, laboratory testing, fertility goals, and an evaluation by a qualified medical provider—not simply a single testosterone number.


Clomid for Men: How Does It Affect Testosterone?

Clomid (clomiphene citrate) belongs to a class of medications called selective estrogen receptor modulators, or SERMs.


Although clomiphene is FDA-approved for certain forms of female infertility, clinicians may prescribe it off-label in appropriately selected men.


In men, clomiphene can alter estrogen feedback at the hypothalamus and pituitary. This may increase the release of LH and FSH, which can subsequently stimulate the testes to produce more of their own testosterone.


Why Might Clomid Be Considered?

Depending on a patient’s individual circumstances, a provider may consider clomiphene when the goal is to:

  • Support the body's endogenous testosterone production

  • Increase testosterone without directly administering testosterone

  • Maintain stimulation of the testes

  • Consider fertility goals when developing a hormone treatment strategy


Clomiphene is not appropriate for every man, and individual responses can vary.


Enclomiphene for Men: How Does It Work?

Enclomiphene is the trans-isomer of clomiphene and also acts as a selective estrogen receptor modulator.


Like clomiphene, enclomiphene is intended to influence the hormonal feedback system between the brain and testes. By reducing estrogen-mediated negative feedback at the hypothalamus and pituitary, it may increase LH and FSH signaling and support endogenous testosterone production in certain men.


Importantly, enclomiphene is not currently an FDA-approved drug for the treatment of male hypogonadism. When considered in clinical practice, patients should understand its regulatory status and discuss the potential benefits, limitations, risks, and alternatives with their medical provider.


One of the biggest differences comes down to a simple question:

Are we replacing testosterone, or encouraging the body to produce more of its own?

With TRT, testosterone is administered directly through treatments such as injections, topical preparations, or other formulations.


Clomiphene and enclomiphene take a different approach. Rather than supplying testosterone directly, they influence the hormonal signals involved in the body's natural testosterone production.


This distinction can be particularly important when fertility is a concern.

Exogenous testosterone can suppress LH and FSH signaling and may reduce sperm production. Because SERMs stimulate rather than suppress gonadotropin signaling, a qualified provider may consider them in certain men when preserving fertility is an important treatment goal.


Neither approach is automatically "better." The appropriate treatment depends on the individual patient.


Who May Be a Candidate for Clomid or Enclomiphene?

A hormone optimization evaluation may be appropriate for men experiencing symptoms potentially associated with low testosterone, including:

  • Reduced energy or stamina

  • Decreased libido

  • Changes in sexual function

  • Difficulty building or maintaining lean muscle

  • Increased body fat

  • Reduced motivation

  • Changes in mood or overall sense of well-being


However, these symptoms are not specific to low testosterone. Sleep disorders, thyroid abnormalities, medication effects, metabolic conditions, stress, depression, nutritional deficiencies, and other medical conditions can produce similar symptoms.

That is why comprehensive evaluation matters.


Fertility can be an important consideration for men evaluating hormone treatment.

Traditional testosterone therapy can suppress the body's production of LH and FSH, which can reduce intratesticular testosterone and sperm production.


For men who currently want children—or who may want children in the future—this should be discussed with a medical provider before beginning testosterone therapy.


Because clomiphene and enclomiphene act on the body's hormonal signaling rather than directly replacing testosterone, they may be considered in certain clinical situations where maintaining endogenous testicular stimulation is desirable.

Are There Side Effects?

Yes. All medications have potential risks and side effects.


Clomiphene and enclomiphene are no exception. Potential adverse effects can vary by medication and individual and should be discussed with your prescribing provider. Appropriate follow-up may include symptom assessments, laboratory monitoring, and adjustments to treatment when necessary.


Seek prompt medical attention for concerning or severe symptoms.

Hormone therapy should always be medically supervised rather than approached as a one-size-fits-all solution.
